Application Forms

Step 1: Download the Application Package

Important: The form can either be printed, signed and sent by email or fax, or it can be signed and submitted digitally. To submit digitally, please save the PDF form in an easily accessible folder on your computer. Open it using Adobe Reader or Foxit Reader, then fill and save the form.

Step 2: Crown Eligibility Review

Complete the following documents with the assistance of Defence Counsel:

1. Program Application Form
2. Application Waiver (Part I)

Step 3: Submit the Application

Send the completed forms to the prosecuting agency for review. You can submit the form in 2 ways:

1. You can choose to submit the form digitally by clicking on the “Submit Application” button within the PDF form.
2. Print and Fax the form to one of the fax numbers listed below.
